Basically the veins (Corpora cavernosa) that normally swell to cause an erection are removed and replaced with plastic tubes that will fill upon 'command' and swell causing an erection. The command us you pressing a pump inside your abdomen.

A cousin had it done. It became infected while on a cruise and when he returned the MD said a few more days and he need to be castrated.
